Is it me, or is the passenger seats meant to be offset back from the dash 11" more than the driver seat?

That is the way it was designed. If you notice the engine is not centered in the van. It sits closer to the passenger so it was necessary to move the passenger seat back to provide any reasonable legroom.

In addition the drivers feet are where the heater box is on the passenger side.
1972 E-200 3 on the tree, part of the family since 74!

In my van, it looks as if there are factory holes for moving the passenger seat forward to be parallel with the driver's seat. Sometimes I wish I could chat with the engineers to ask "what the hell?!". I was surmising that the seat was pushed back due to dog-house removal. This is interesting info and thanks guys!
